Have you visited a health care provider on the list? Do you have feedback you’d like to share? Positive or negative, we’d love to hear it. If you’re willing, please use the form on this page. In most cases, we use a reparative resolution model to address harms caused by providers on our list. This means: (1) We will contact the provider with the information you provide (*you can be remain anonymous*) and (2) we offer education to help them do better. The ultimate goal is to have as many affirming healthcare providers in our state as possible.
You are welcome to be part of this reparative process, or not. It’s completely your choice.
In some cases, the harm is too severe and we will choose to simply remove the provider from our list without participating in a reparative resolution model.
